Notice of Privacy Practices

This notice describes how your medical information may be used and disclosed, and how you can access this information. Please review it carefully.

Who We Are

Southcentral Foundation (SCF) is a non-profit Alaska Native–owned and managed healthcare organization incorporated under the Cook Inlet Region, Inc. (CIRI) and authorized under PL 93-638. We are covered entities under the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and are required to maintain the privacy of your protected health information (PHI).

How We Use and Disclose Your Information

SCF may use and disclose your PHI for treatment, payment, and healthcare operations without your specific authorization. Treatment purposes include coordinating care between your primary care provider, behavioral health consultant, pharmacist, and specialist teams. Payment purposes include billing Medicaid, Medicare, and private insurance. Healthcare operations include quality improvement activities, training programs, and accreditation reviews.

Your Rights Regarding Your Health Information

  • Right to inspect and copy your PHI maintained in your medical record
  • Right to request that SCF amend your PHI if you believe it is incorrect or incomplete
  • Right to receive an accounting of certain disclosures of your PHI
  • Right to request that SCF restrict how it uses or discloses your PHI for treatment, payment, or healthcare operations
  • Right to request that SCF communicate with you in a certain way or at a certain location
  • Right to receive a paper copy of this notice upon request
  • Right to opt out of receiving fundraising communications

Our Duties

SCF is required by law to maintain the privacy of your PHI, provide you with notice of our legal duties and privacy practices, and follow the terms of the notice currently in effect. We reserve the right to change our privacy practices and the terms of this notice at any time, provided that the changes are permitted by law.

Uses and Disclosures Requiring Your Authorization

SCF will not use or disclose your PHI without your written authorization for: marketing purposes; sale of PHI; psychotherapy notes (except in limited circumstances); and most uses and disclosures of your PHI for research, public health, or other purposes not listed above. You may revoke an authorization in writing at any time.

Disclosures Without Your Authorization

SCF may be required or permitted to disclose your PHI without your authorization in certain situations, including: as required by federal, state, or local law; for public health activities; to report abuse, neglect, or domestic violence; for health oversight activities; for judicial and administrative proceedings; for law enforcement purposes; and for other legally required purposes.

How to File a Complaint

If you believe your privacy rights have been violated, you may file a complaint with SCF or with the U.S. Department of Health and Human Services Office for Civil Rights.
