Skilled Nursing Visits

For elders, those with complex chronic conditions, and those facing transportation or mobility barriers, leaving home for every appointment is not always possible. The Nutaqsiivik program ensures that the quality, relationship-centered care SCF is known for reaches every customer-owner — regardless of circumstance.

Home visits are fully coordinated with your assigned clinic care team, ensuring that what happens at home stays connected to what happens in the clinic — and vice versa.

Who qualifies for Nutaqsiivik?

Alaska Native and American Indian customer-owners of Southcentral Foundation

Elders (55+) who benefit from in-home care support

Individuals with complex chronic conditions requiring ongoing monitoring

Post-surgical or post-hospitalization patients requiring skilled nursing

Those with mobility, transportation, or accessibility barriers to clinic attendance
