Your Integrated Care Team
Your primary care provider and team includes a full-time RN case manager who focuses on care coordination, and a case management support coordinator who schedules appointments.
Additionally, specialized members can be added to your team as your healthcare needs change. For example, your RN case manager can refer you directly to a pharmacist, nurse midwife, chiropractor, or physical therapist.
