Southcentral Foundation Receives
HRSA Silver Quality Leader Award
Awarded by the Health Resources and Services Administration for ranking in the top 20% globally in preventive screenings, diabetes care, and relationship-based health access.
The Health Resources and Services Administration (HRSA) has officially designated Southcentral Foundation as a Silver Health Center Quality Leader, recognizing the organization's outstanding achievements in clinical quality, preventive screening accessibility, and relationship-centered native healthcare clinics across southcentral Alaska.
The Health Center Quality Leader award recognizes health centers that achieve top clinical performance in key metrics among all HRSA-funded health systems nationwide. The Silver award signifies that Southcentral Foundation ranked in the top 20% of clinics globally for clinical quality measures, reflecting the success of the Nuka System of Care.
National Clinical Standards
Placing Southcentral Foundation in the highest tier of community health systems globally for preventive healthcare interventions.
Relationship-Centered Success
Proving that multi-disciplinary care circles and custom scheduling panels generate superior medical outcomes compared to standard clinical networks.
Specifically, Southcentral Foundation demonstrated exceptional leadership in preventive screening rates (including mammography, colorectal screenings, and childhood immunizations), chronic care management (maintaining target blood pressure and glucose levels), and timely entry into prenatal support.
Commitment to Outstanding Community Outcomes
By coordinating patient care circles that bring primary doctors, nurse coordinators, case managers, and behavioral health consultants together in one room, Southcentral Foundation customer-owners bypass standard clinical wait cycles and receive comprehensive care in a single visit.
“This silver leader award is a testament to the dedication of our clinical care circles and the trust that customer-owners place in their integrated care teams,” said a clinical program leader. “In Nuka, we build relationships that encourage customer-owners to actively direct their own wellness journeys.”
